Name Field - Confusion

In my passport Surname is not there (I never mentioned my surname in any of my documents) and only Given Name is there.

In Generic Application Form what should I mention in Surname field (as per instructions I need to mention as per passport, but in passport Surname is not there) Please advice. Can I mention as NA?

Yes. As you cannot leave it blank, put N/A. Give your full name as it is in your passport, in the column "Given Names"......
They will compare with your passport copy.
