Name Change

Is it necessary to state that you had a name change after getting married more than 20 years ago and you've been using your husband's surname since then? Should I still indicate that my maiden name changed? Isn't this obvious when I submit my marriage certificate? Thank you for any help.

If you are answering a question about whether you've ever used another name you should say 'yes' and give your maiden name. Your marriage certificate will constitute documentary evidence of same.
