Of course, not all the information is still valid but most of them still provide useful insight(from getting AOR upto submission of passport) following completed forms requested by CIC forms forwarded to CIO NS.

Depends on the clinic. SLEC charges P5,200/pax. There are some additional charges if they need additional testing, not sure about those.

I had to have additional blood test because I had more than one ear piercing. It costs around 2,700 pesos extra.
