They did not specify specific size or format but I guess just make sure that it is big enough to be readable. I used font size 15 (Arial) just to be sure and of course white sticker paper (there are colored ones, even neon!)

As for format, just follow the usual name, complete address scheme you usually see for mailing labels.

The guide says to fill out the payer information on the receipt and attach the BOTTOM PORTION (COPY 2) of this receipt to your completed application.

As long as it is the same address you want now to receive any mails from CIO. In my case, for MPNP, I used my sponsor's address in Winnipeg in order to speed-up receipt of LOA but for my CIC application, I opted to use my Philippine mailing address since my VO is Manila for faster exchange of documents.

There's no bottom portion! Print, fill-up and attach the COMPLETE receipt with your application. Do keep a copy of the receipt for future reference.
