Mistake in filling medical form plz help

Salam alekum all,

Just made my medicals today,went smoothly insha ALLAH,but i made a silly mistake in filling the form regarding my name,lets assume my name is a b c d, wgere a is furst name, b second ,c third , d is surname.in my passport all written together a b c d, but in my genetic application i wrote family name as d, given name as a b c, in the midical request from vo they wrote family name as a b c d, given name blank, and i wrote in the fill up forms of the clinic ,family name b c d, given name as a. I told the doc about it,sge said she will write a note that name appears as passports and she will wait till sunday for my confirmation plz guys advise me

becuz i forgot how my name appeared on the generic form i filled a year ago and I have a copy of it so I told her i will check that paper and call her again to clarrify.

why did u make it so flummox?

in your passport all written together a b c d
in generic application you wrote family name as d, given name as a b c
and in the fill up forms of the clinic ,family name b c d, given name as a

now go through by the name you have written in your generic application.as far as medical concern let it be go as is ,do not make any more confusion which will cause delays
