I would appreciate your support, I have received the Medical Forms for me and My family, all are fine except mine, as my surname was splitted between my first name and surname in the form so my actual first name was not written at Medical Form. I have went to medical Center , the feedback was that names would be written as medical form not as Passport but passport would be attached.
what is your advice? shall I proceed with this Medical Form?
I have sent an email to Embassy, but as always I have to wait weeks to get Feedback.
