medicals - anyone with similar experience

My son has been asked to undergo medicals within 90 days as a condition for extension of study permit granted by CIC. I have also received my MR for my PR application where he has to undergo the medicals. I understand MR is valid for one year and I intend to let him get his MR done once. But my question here is - Which form to use. Whether to use IMM 1017 received for the study permit or the IMM 1017(E) received for PR? Anyone had this situation before? Please suggest.

As long as I know if you have done medical once than you don't have to do it again if you need 1 within a year. So doesn't matter through which ever form he's going for medical.
