Medical Question..

My wife went to get her Medical Exam done today in Louisville Kentucky. She brought the pictures and form or piece of paper for the doctor to fill out. Isn't she supposed to get the form she brought back and we mail it with our application? The doctor today kept hers. Thank You

They fill out a form and the green copy is hers. It has on it one of the pictures that she took with her. She needs that to send in with the application.

She needs to get back to that doctor's office ASAP and get the right form. As others have said, it's a green form, and they stamp it with a maple leaf. IMM 1017 EFC

The applicant doesnt ever hear about the results of the exam. The doctor is supposed to give the applicant the form so that it can be sent in with the app. The results go straight to the appropriate CIC center - in the case of a US applicant - Ottawa.

She should not be waiting for results to get the form.

heatherusa is ABSOLUTELY correct.

I received my form right before I paid the fees at the doctor's office, and before I walked out the door. YOU NEED TO LEAVE WITH THIS FORM.

I had blood taken and the x-ray done at the office. They obviously wouldn't have the medical results for several days, but said they'd forward them to Ottawa once they came in.

I've yet to hear of someone not receiving this form, so something definitely sounds amiss. You should press them on this. You may be waiting on a form that will never show up in the mail. It's not worth the risk.

My partner did his medical in Minnesota, and we asked our doctor the same thing. We were not given a green form (he showed us what we needed). All we had was the form from the application package.

He gave us a copy 2 of the form we had, but did not sign it, he put his stamp on it and said that he would not sign anything without getting results back. The results would be mailed to Ottawa and held there. He would provide a letter saying the medical was done along with the unsigned cop of the form

Just found out Buffalo has his medical results... and all is well.

Was copy 2 a green form with the picture on it?

Nope, it was a white form that he had printed off the CIC website. It wasn't the one in our package that we brought to the office. We were very skeptical when he gave us the copy. He did show us what this "green form" everyone else is talking about looked like though. He said we should have gotten if from the Canadian Consulate... but we did not read that anywhere in our guide. I'm assuming what he gave us was a PDF version of this green form? It did have a spot for my partner's picture, which he attached, he did stamp it (red stamp with maple leaf and his number) he just would not sign it. He said that the unsigned copy is what we should submit with our application, and the other copy he would send to Ottawa.

Ok now i am even more confused..sorry..haha..Copy 2 of Appendix C is this a form my wife brought, or a form from the doctors office?
