medical green slip? confused... please help!

Hi guys,

so my hubby checked his email today and to our surprise we got an email from the Damascus office.
this is what they wanted:


- If you have already undergone a medical exam, please provide us with your medical report (green slip). If not, you can visit one of our Designated Medical Doctors to pass a medical examination without the need to receive a medical form from our office. List of the Designated Medical Doctors is attached.


My hubby already did his medical a few months ago and we sent the receipt of it along with the application, however we don't have any green slip? I don't know if we should just send the original white receipt we got from the doctor or if he has to go for another medical again? It hasn't been a year yet (will be a year in November) so I know his medical hasn't expired... But what I'm worried about is that he never got a medical "report" it was simply a white receipt that showed he had paid for the medical and that it was for the permanenet residence application. Were they supposed to provide us with something else?

And I'm a bit lost as to what they mean by "If not, you can visit one of our Designated Medical Doctors to pass a medical examination without the need to receive a medical form" ? Can anyone please offer some advice on what we should do...

I just want to send everything in ASAP, so if anyone has any suggestions, please reply :)

When my husband did his medical they gave him yes, a receipt for the payment, but also a greenish colour carbon copy page of what they send in with all his results so that they can match up the data with the sponsership application when it comes in.

Perhaps call the dmp's office where you did the physical? They may have it on file and just forgot to give it to you?

Did your husband visit a Designated Medical Practitioner to get his exam? If so, I would suggest that you contact the DMP who did the exam and find out which Regional Medical Centre s/he sent the exam to. Then fax that RMC, verify that they have the results of your husband's exam, and ask them to forward the results to the visa office that is processing his application. Then contact the visa office again with a copy of your faxed correspondence with the RMC. That should do it.

This is basically part of a form letter, I think. Sometimes (like for certain types of applications, or for medical re-dos for spousal exams [for example with us after our appeal]), the embassy will send a form that you take to the DMP's office. Mine was a greenish form - but the receipt I got from the DMP both times I had an exam done (for my initial ap and for the redo after appeal) as just as your husband got . . . a white, unofficial receipt showing that I paid $XXX for an immigration medical exam. The DMP sends the results to the RMC - the applicant does not get results and does not need to provide the visa office with anything other than proof that they underwent examination. Even though we got a "greenish" form from the visa office for our medical re-do, the DMP kept that - it was not returned to me to send to the embassy.
