Just a quick background...
AOR: June 8th, 2019.
I did my Medical on May 3rd, 2019.
I got PFL in early February 2020. I was given 7days to respond, responded before the deadline
May 28th, 2020: My profile changed to “You do not need a medical exam. We will send you a message if this changes.”
June 4th, 2020: My profile now reads “June 2,2020. You passed the medical exam.”
And I DID NOT REDO my medical.

My question: Does that mean my application has/will soon be approved?

I spoke with CIC agent few minutes ago and she said my eligibility has been met, just waiting on my background check and a final decision will be made.
 
Case officer has the ability to extend the validity of medicals in all forms of applications.

My case similar to yours. I did my medical in 2nd of May 2019. AOR 17th June 2019.
My medical got extended on April 2020, and my application got approved soon after that. So, I believe it’s a good sign.

When did they finally approve your file? Are you in the USA because I am and my visa office is Montreal.

I’m not sure when exactly. First everything passed in the end of April and then Approved this week. The exact dates I’m not sure about, and yes I am in the USA and my visa office is Ottawa

Have they asked you to submit your passport because I thought they were only dealing with inland applicants for now.

No they didn’t. I only got to know per a phone call. As far as I know they aren’t issuing any COPR for outland at the moment.
try to contact them to know your status.

Spoke with them on last week and I was told eligibility met, but they are still working on my background check.

No, my PFL was not related to my medical. It is related to one of my job experiences, I submitted a notarized letter written by myself regarding my work experience. They requested an original letter from my former place of work on a letterhead paper, bank statement, etc. just to verify my claim.

Did u get COPR in the end ? May I know when u got it after medical extension ?
 
Yes I did. My medical was extended June 2nd, 2020; I received PPR in September 23rd, 2020. COPR October 19th 2020.
