Hello Everyone,
I live in Mississauga. My wife is Chinese living in China, Zibo City, we married last year in October.
Now in the process of working on the immigration forms, I am a bit lost to find Appendix C,,.
I only see Appendix A,, the Applicant check list and we follow that and the check list.
Can anyone help?,, PLEASE,, how to get the from for medical check, as far as I know the IMM1017 form is not in use anymore? then what it is,, Appendix C,? I understand there are medical centers in China designated by the Canadian Immigration, they told my wife no forms no exam.. where we can get the forms for the medical exam?

Hi, Apparently the rules changed, effective November 1, 2012. The following is from the CIC website:

Operational Bulletin No. 471

"Coinciding with the implementation of eMedical, will be the release of GCMS 4.0 which will include new medical screens. As of November 1, 2012 the new medical examination forms will be disseminated to all panel physicians. From that date, panel physicians, eMedical-enabled or not, must forward all information using the new immigration medical examination forms." ...

"With the release of the new GCMS 4.0, it should be noted that all previous versions of the IMM 1017 form as well as the Appendix C will no longer to be used. With this release, there will be two newly-revised IMM 1017 forms:

IMM 1017: Medical Report: Client Biodata and Summary (not available on our website as it will be GCMS-generated along with medical instructions)
IMM 1017B: Upfront Medical Report: Client Biodata and Summary (not available on our website as panel physicians will have these forms in their office. With the implementation of the new IMM 1017B, Appendix C will no longer be required)"

This explain a bit though, in the existing principal applicant check list Appendix A, form IMM 3903E, there is check box#14; PROOF OF MEDICAL EXAMINATION,, how I deal with it,, leave it not checked??
Please answer, is this Appendix A, IMM3903E a valid check list for the applicant outside of Canada ?

After the Medical exam is complete the Dr. will give your wife a form (IMM1017) that is the "Proof of Medical Exam" (also sometimes call the "receipt") and this is the form referred to on the checklist that you will send with your application.
 
If the physician absolutely insists you must bring Appendix C or they won't do the exam, you can download it here: http://www.cic.gc.ca/english/resources/publications/dmp-handbook/pdf/AppendixC_e.pdf

It's just basic information. Although as mentioned this form is now NOT required, but I guess lots of physicians around the world didn't get the information properly from CIC.
