I have a question??? My wife is going to do her Medical check tomorrow and I'm wondering what paper work/forums does she need to bring. Is the Appendix C one of them?

I just want to make sure we know exactly what she needs so she will not need to go back.

When my family had theirs done all they did was bring the passports along, and they filled out the forms given to them in the medical office. Once you get the medicals done they issue you a form with a barcode on it that you attach to your application for CIC to review the results once they check.

In contrast to dj heckford, my wife and step-daughter needed their passports, 4 photos, immunization records, clinical history, and payment receipt from 3 days prior.

She can bring her passport and she will also need a 2x2 photo. When she gets there she is to tell them she is there for an upfront medical under the family class sponsorship for Canada.
