Actually before sending the application I talked to CIC call center about my Birth certificate and they said that "Certificate of Date of Birth" provided by Consulate General of India is acceptable. But now they rejected my application for the same reason.

Now, Can I send the same application set back to them along with the Affidavit for Birth Certificate?
or do I need to send fresh copy of application again?
As per the CIC Immigration guide, Police clearance should not be older than three months during submission. But because of this delay, my police clearance is more than three months old. Will they accept it or i need to take new police clearance?
Someone please clarify my doubts.

Sorry to hear that! When did your application came back?

Just add that affidavit and send application again. Police certificate is not mandatory to sent straight away, you can always send it later when you get a new one. Just write a latter of explanation for that.

I got it today. Glad to hear that I can send the same documents again. I will make my DOB Affidavit tomorrow itself and send all the applications again along with one more explanation letter. What about the time frame? Will they take a month again to send the AOR? If not, how long would it take? your reply is greatly appreciated!

Today I also received my application back because of same reason (there was no other problem in application). I have few questions if any one can help -

1. I live in montreal, where can i get affidavit done. Is it notary or lawyer? If any one took affidavit already please give refrence for the notary / lawyer.
2. Same questions listed above -

Now, Can I send the same application set back to them along with the Affidavit for Birth Certificate? or do I need to send fresh copy of application again?

You can get it from any notary or commissioner of oaths. a lot of immigration consultant are also commissioner of oaths. Good luck! :)
