You still need to pay. $550 is application fee. $490 is residency fee!!
 
Hi,

I am May 21 applicant with upfront medicals. Haven't heard any thing yet from CIC. I know couple of other may applicants are there who didn't heard. I have started to loose patience now. :(

That is what I thought,my MR also has biodate,I think that is the different one.

I went yesterday to the University of Ottawa clinic. They have eMedical so you don't have to take photos with you as they will be taken they by a digital camera. Just take your passports, permits (temporary residence documents), and print out the medical requests you received from CIC.
You will be dealing with Dr. Vincent Nadon. He is a good doctor but we had to wait sometime to see him. We spent 3:30 hours in total although we arrived an hour before. I took the day off anyways.
After you do your physical tests, you have to go to the lab on Kent St. for the x-ray and the blood test. Try to go before 3:00PM otherwise you will have to go the next day for the x-ray (yes, we did the x-ray today, the following morning).

In total, we paid $810 for 2 adults and 1 3-years kid + $15.xx for the blood test for the 2 adults (not required for kids) + $100 for the x-rays (again, not required for the kids).

They said it should take 1 week for the results to be sent.

What documents Dr John C Kuhlmann requires for Medical? Can any one please guide

Hi Frnds - Need Help, While sending the BIO data form - Do I need to attach a document or just send that tabular format in the e-mail is suffice?
Please advise.

For the Biodata form you have to print the form, fill it out, scan it and email the form back to CIC.
Or use the fillable PDF form you can find on my website, fill it on your computer, save it and email it to CIC.

They will need Passport, Medical request form (sent by CIC) & Cash(140+36: for adults). As per CIC you should have previous medical history reports if any.

Thanks a lot. I am in Canada for last 3 years. Do I need to take history from my family doctor? So far there is no issue with my health
