Manager Reference Letter format feedback request

Dear Forum Members,
My Manager from previous company "X" has agreed to write the reference letter, with the job responsibilities, but it might not be on the company letter head. Also, the HR of the company "X" will provide a letter, stating my start and end dates, Position, Salary and a confirmation that it was a full time, on the company letter head. Therefore, below is the format, that I have come up with. Please provide feedback for the same.
