Letter of Reference - Substantial number of the main duties?

So many people got rejected because of letter of reference and most of them have enclosed detailed letters with job description.

Question:
How do you call a letter meets "substantial number of the main duties"? Let's say there are 6 items listed on NOC page, how many items should you cover in your letter?

Additionally, I want to share a lawsuit about copy-and-paste of NOC description:

You can read from the judgement that visa officers will be skeptical about your letters if the description of your jobs is only paraphrased or look exactly the same as that on NOC page.
