hi everyone

getting ready to send my papers off on monday..I probably asked this question before but just want to confirm since I have read my papers 50million times over scared I may miss something. The forms example "use of representation" or "dependent" if we did not use or have either do we still have to put the forms in the pile? or on the checklist can I just write n/a beside it?
 
you do not have to submit the form if you do not use a representative...

I'm not sure about the other form ("dependent") that you are asking about...

Thank you jamgirl and the other sorry I meant "additional dependants IMM0008DEP" and Statutory declartion of common law union IMM5409" theses also are not applicable to us so I just leave it out all together? do I have to write a comment on the document checklist? Also, if I include my notice of assessment do I still have to include "proof of employment & proof of income" # 18 on the checklist as well?

I confirmed both of the questions you are asking with CIC before I sent my application to them and I was told that I did not have to submit the forms; just write N/A on the checklist if you do not need to submit the forms. Further to this, when I spoke to them about the use of a representative, they suggested we submit the form since I assisted my husband with filling out the applicant's application to make sure he understood everything. We also completed and submitted the use of a representative form so that all communication would come directly to me from CIC. I was concerned that mail may go missing in Jamaica if anything was sent to my husband so I personally felt better being my husband's 'representative'.

So what you did was just fill out 1 copy of the Use of a representative and included it under the "sponsored persons" application?

Yes, my husband completed form IMM5476 and completed sections A, B, & D. He signed as the applicant and I signed as the representative.

On form IMM0008, page 2, Contact Information section point #1 Current mailing address, it states:

- if you wish to authorize the release of information from your application, to a representative, indicate their email and mailing address(es) in this section and on the IMM5476 form.

Not at all you are being such a wonderful help to me right now and making me less stress as I triple check everything. :) Did you include

Yes, in addition to the Option C Printout, you will need to provide an original letter from your employer stating your period of employment, salary, and regular hours per week.
 
I may be a little late and u may not even see this but congrats sammy hope u have a safe flight and that your wedding day is filled with magic and joy...enjoy every moment of it try not to stress about anything

I didn't submit anything else for proof of employment and got approved for sponsorship so I think you're good with what you have.
