The new ones who got PPRs today, do you have a Annex A form in your PPR letter?
They asked me to fill the form for confirming mailing address purpose, but this form was divided into two pages when I printed the letter. Is that OK?

That's right. Just print the whole email message and fill that Annex A section, courier it along with passport and photos.

Before printing, click on pre-review and print as a smaller size?
