My friend got AOR today. Her AR was April/6/2020. Inland and AINP. Four family members. It is not temporary AOR.

Yes you have to fill that out print it out and sign it. If you don’t have enough space add extra sheet of paper and put your name, DOB and uci on that sheet and fill all the history over there and then at bottom sign it. For the gap you can mention it as unemployed. Make sure you fill it and then print and sign.
