Include the form and put: "n/a (not applicable)".

You could also put the sponsor as the representative, that way they can also enquire about the application too.

It's also important to note that if you include the sponsor's mailing address as the principal applicant's mailing address (with a different address for the principal applicant's residential address), you MUST include the Use of a Representative form making the sponsor the representative, or the app will be returned. Happened to me.

but currently we live in the same house as we are married . our residential addresses are same . plus , where on that form will i be writing N/A . and i have another question too as i have started filling out forms by myself i ve alot of problems . if you could help me out . that will be so nice of you ?

There is no need to include the representative form if you will not have one.

I did not use one and was approved and landed last year. I did put N/A .

Thank you so much . One more question . Where to put N/A in that form . In all fields or just one will suffice

I did not use a representative.
So, I did not include any of the representative forms with the application package. I did, however, write "N/A" quite clearly on the checklist in the left margin next to each time the checklist referred to the representative forms.
