Is the IELTS test result date at the bottom of test report forum?

Hello, to enter the EE pool, there is questionnaire. And there is a question about the test result date, is it the one at the bottom of the test result form?

I know the test date is on the top. But in my form, the date at the bottom is only 7 days later than test date (I remember test result should be 14 days later than test date from IELTS center)?

Does anyone know about this?
