is it true that medical exam and application for PR goes together?

somebody can help me!
a) is it true that the moment you received the application forms from the CIC
for PR medical form is included?
in my case, i received only:
1, IMM 0008, schedule 1 Background/declaration
2. IMM 5476
3. IMM 5406
4. IMM 5490
5. Appendix C Photo Specifications

If it is a spousal/child application, the DMP has the medical forms.

1. what do you think the reason why we don't have a medical form enclose
in our PR application when they sent to us?
2. Visa offices has different rules/regulations/specifications for "Family Class"?
3. Can we voulontarily go to the MDP for medical exams who is CIC Accredited ?

As it is a spousal application, the DMP has the forms, so you go to the DMP, read the instructions, the overseas office will not send you medical forms.

but i just want to clarify that my wife is a CAREGIVER in mississauga,

1. Are we still belong to the same time line 6-8mos in CIC Manila for the
"FAMILY CLASS" or different from other cases? if we can now have the
medical exam together with my 3kids?

In the case of LCPs the family overseas is supposed to complete their portion of the application and return ti to Manila. Since the processing time in Canada is 5/6 months after application, it maybe that Manila is delaying sending out the medical so that it doesn't have to be re-done. But who knows. as i reviewed the communication Application/Instructions sent by the CIC manila, so far, i completed all the Documents Required, all were Original and has sealed by NSO ( National Statistic Office ) and NBI ( National Bureau Investigation) . Only the first 3 pages of the Passport were photo copy as they required, I sent also the proof that our relationship is genuine as such before and Wedding pictures,Spouse/Partner Questionnaire
But unfortunately, i was unable to read clearly that before the checklist shown below, there were i think i miss read that portion , ( include 3 recent photos of each Family member)
i did all the requirments of Appendix C Photo specifications.
since it's almost a year now when i returned/submitted my application to the CIC Manila,
1. CAN I STILL DO SENDING OUR RECENT PHOTOS WITH MY KIDS to CIC
Manila?
2. thus i will include my Mother/brothers/sisters/in-laws ? whether or not
accompanying in Canada?
