Is anyone ever denied a PGWP if they meet the eligibility requirements?

Has anyone ever heard of someone being denied a post-graduation work permit if they meet all the basic requirements?

Does it work as a lottery, like the H1B visa in the USA or is it pretty much guaranteed if you've done a degree at an eligible Canadian university?

There is never a guarantee for a visa. Some people got it denied even if they had all the required documents etc.

And no, it isn't a lottery. If you have all the necessary documents and have all the requirements it should be fine as long as you apply within the 90 days after graduation.
