Inland sponsorship medical question

I am at a complete loss here.

We hired an immigration consultant to help out with the paperwork because hubby said I was too high strung about it all. I admit, I am a bit OCD, but that is another story. LOL

So, I went and got my medical done yesterday along with blood/urine/x-ray, you know, all the standard stuff. Here is where my question comes in.

For an inland application, what is supposed to be turned in WITH the application regarding medical? There seems to be more than enough confusion going around even as far as my consultant goes. The consultant wants me to get the little 'green' 1017 form, however, that is only given to those who turn in Appendix C at time of medical and THAT is only for OUTLAND applications. I am doing INLAND and brought in the 1017 form for INLAND applications.

Can ANYONE tell me which form, if any, I am supposed to have attached to my application? Yes, I know I have a consultant, and NO they can't answer this. They want me to return to their office, and get Appendix C to gain OUTLAND form 1017 to attach to my application. Which would mean that I need yet another 2 immigration photos (have already done 6) and it makes absolutely NO sense to return to get outland forms for an inland application.

HELP ???

First thing, you're wasting your money on the consultant, it's a very easy app to do yourself
Second, when I did my medical I brought 2 pics to the x-ray
2 to the urine and std
And 2 to the med doctor with the form to fill out at the doctor
When you send your app in you send only the receipts with the 2 or so
Requested pictures hope this helps:)
P.s when it comes to this it's alright to be OCD
It's your future on the line here:)

So you only turned in your receipts from the docs offices' stating that you have obtained your medical and that was acceptable? If that is the case, this is awesomeness! @ ! ;)

Does anyone have any ideas? I know there has to be a lot of inland applications going on here. What did ya'll send for your medical part? ::)

I submitted my medical with the application.

I went to the doctor with the form and photos.

All i got back was my reciepts. The doctor advised that he would send everything into CIC on my behalf.

I sent in copies of my reciepts.

Hi Alurra71,

When you go to the Doctor they will give you a form - which confirms that they carried out a medical exam. You send this form in with your application - it does not contain results. The results of your medical are never seen by you - the Doctor sends the results directly on to Ottawa where they are entered into "the system" and can be accessed by the immigration officer handling your file. I have gotten approval in principle and I am currently waiting for stage 2. I didn't send in the medical with my application so it will delay my application slightly.

That is the form that I took to the doctor yes, and that is the one they kept and said they would send it directly on to where it needs to be. If that is the case, then the ONLY thing I have now is the actual receipt from the doctors office. The one where I paid. So that is what I need to send in with my application?

Yes send in a photocopy of the receipts and a letter stating that the Dr didn't provide any further proof of your medical.
