Inland Open Work Permit Application

Hi everyone, I'm sponsoring my husband via inland application and he is also going to apply for an open work permit at the same time. When he includes the OWP application does he only have to fill out the IMM 5710 (application to change conditions, extend my stay or remain in Canada as a worker) or include everything for that application such as check list, the other forms and photocopies of his IDs?
