Indian Applicants who received CSQ recently please guide

I have started this thread specifically for Indian applicants who applied for QSW and got CSQ. PFB the list of documents which i think are enough to get CSQ. Kindly advise if there is anything left out.

1. Birth certificate : Would a notarized self affidavit would do or need original birth certificate only ?

2. Reference letters from companies on company letter head

3. Notarized IELTS result document

4. Form 16 or 26SA to show the tax and refunds

You must produce birth certificate. I frankly find it astonishing that Quebec asks for this. A passport is a de facto standard for establishing a person's identity and age throughout the world. They must not trust the passports.
