Indemnity form received from CIO for Processing fees refund after rejection

Recently I got a soft copy of "Indemnity form" the way to refund processing fees for the rejected files.
But the fact is that, this is somewhat complex to fill up..
If you guys received such Form to get the processing fees refund, pls. HELP ME....
