Immigration office by error refused to give open work permit for my husband (I am an international student)

Hello,
I am an international student and my husband is here in Canada as a visitor. we applied in November, for open work permit for him and got a refusal letter due to the reason that in September (before couple of months he even had not an account then) they somewhere sent medical exam request which we did not received. We did Medical Exam and reapplied for work permit but we got refusal again and the reason was his visitor status that they considered to be expired in February . In case his status expired in the end of April (cause we were back to our home county in autumn and when we returned the border officer gave him 6 months; They advices in refusal letter to apply for status restoration but his does not need to restore) and we requested extension cause of COVID-19 he could not leave. So as you see their is no breaking any rules but because they did not take attention of the submitted passport where was a stamp on it and only checked last extension we did before latest update in his passport, they refused. Moreover, we also applied for visitor status extension and waiting for response.
I do not know how let the immigration office know to check his passport while considering his extension? And if I let them know how are the chances that they reconsidered their mistakenly made decision about work permit?

Can you post the full refusal letter removing any personal information (e.g. name)? Can you provide the full chronology of your husband's time in Canada and applications since it's a bit difficult to understand that from your post? (i.e. when you originally arrived, when you applied for the first work permit, when you received the first refusal, when you left and returned during your trip in the fall, when you applied for the second work permit, when you received the refusal).

Also, check the last entry stamp on your husband's passport and double check there was no date manually written in by CBSA when you arrived.
