How do you put in other names used if you have had more than one previous name? The form will not allow , ; or other symbols and I don't want to put it in a form that they think both names are actually one.

Also, what identity card are they wanting when they ask for a national identity card. I googled it and it says that it could be one of 3: State issued ID like a driver's licence, the passport card, and social security card. I have all three, so do I need to add all three? Or is one better than the rest?

Another question: For children, when they ask for current occupation, do you put N/A, Unemployed, Student, or something else? My first guess would be to put in Student.

N/A. The US doesn't issue national IDs.

Doesn't matter, whichever you think fits.
