IMM5744E Consent Form Problems

Hi all,

I am filling out a consent form so that a permanent resident can apply for my GCMS notes on my behalf. I have filled out their details in Section C, however there is an area to sign below these details called "Signature of Representative".

Who should sign here? Me (the applicant that isn't in Canada) or my friend (Permanent Resident in Canada)?

The representative is the person authorised to act on your behalf. So in this instance, it's your friend.

So I need to fill it out, print it, sign it, scan it again, send it to my friend get them to print it, sign it and scan it back in again?

that seems fairly tedious!
