IMM5476E - Use of Representative form - PGP

I am representing my parents to apply for PGP, so in question 7 , it asks for Name of FIRM and Organisation, their membership id etc.

But as I am a family member and do not own a law firm, what should I fill in? or leave it blank?

What it actually says is "Name of firm or organization (if applicable)"

As every guide says, if something is not applicable to you, write "N/A"

I have another question - Do I need to submit seperate Use of Repreentative form for each of my parents?

To be honest, I'm not sure. If they are able to easily sign forms for you, it won't hurt to have them each sign one.
