1. If my employer's address is different than my workplace (usual case for contractors), which one should I enter?
2. If I worked at multiple locations for the same employer (usual case for contractors), how to I enter that? The form only allows for one address to be entered against an employer. I guess I can split up the employment period by location and enter that, or just repeat the entry with different address, although the later would be weird.
3. How to provide additional employment history? The form only has 3 entries. I think I saw another post where it was recommended that the OP attach an document with a cover letter explaining the purpose. It's not clear on whether the additional document should be another copy of the IMM5257E or just a Word doc?

Same employer is the same business address. Different clients or client locations are irrelevant.

Add an extra page at the end of the explanation letter. Make it clear it is an appendix to the IMM5257.
