IMM 5710/Work Permit - 2 urgent questions

Hi,
My work permit will expire in 2 months and I was filling out the 'Change of conditions/extend your stay' IMM 5710 form. I am stuck at 2 questions.

First one is under section which is in the middle 'Details of intended work in Canada'. Its the first question under that section which states 'What type of work permit are you applying for'? I don't know whether its LMIA exempt or LMIA stream? My employer has only given me an offer letter. Please help here.

Second one is under the same section which is question number 6 stating 'Labour market impact assessment number or offer of employment number'. My offer letter doesn't state any number.

Please help, what should I do if somebody has already filled this form.

What kind of work permit do you hold now? Have you applied for PR? If so, under which program and at which stage is your PR application? What job do you hold in Canada? Do you believe it is LMIA exempt?
