IMM 5604 Declaration from non-accompanying...

for the IMM 5604 form: declaration from non-accompanying parent/guardian for minors immigrating to Canada, I am sure my wife in Costa Rica won't have to fill out right? I am only sponsoring her and no one else, she does not have any children, therefore if that is correct, what do I do with this form, do I still include it in the package to send?

If it's a country or region specific form, fill it in with "Not Applicable" and include it. It can't hurt to send it along.
