IMM 5481 -Sponsorship evaluation form - Q1 and Q3

On the IMM 5481 -Sponsorship evaluation form, for the following questions:

1. Given Name. Do they need just the official name or the nickname goes here as well?

3. Your net personal income for the 12-month period preceding the date of your application.

So if I were to file my application at the end of April, would the period be 2012-04-01 to 2013-04-01? My salary was 3% lower last year compared to this year. So should I be doing the calculations and putting that in there?

Given name - your name at birth. What is in your passport. Only put the nickname if in any of the evidence you are referred to as that name.

Met income for 12 months? Put how much you earnt in the last 12 months. Amount is irrelevant. You could even put 0 and be fine
