IMM 5481 questions

Hi,

I am sponsoring my wife, and we have no children and I am just sponsoring one person, being my wife.

So, in IMM 5481 are the answers like this?

4) 1 (me)
5a) 1 (my wife)
5bi) 0
5b2) 0
5b3) 0
5b4) 0
6a) 0
6b) 0
7) 2 (my wife and myself)


And, in trailer of the form it says "Make sure you include with your application the original option-C printout". "Follow instructions to his effect in checklist". But in the checklist IMM 5491, option C printout portion is no longer there now. It previously used to be there before the checklist was updated. Would notice of Assment be sufficient, as the checklist is now only asking for Notice of Assessment? Please advise.

Yes that is correct, 2 would be the total number

and Yes, now they require notice of assessment instead of Option-C. So Notice of Assessment is perfect.
