Hi everyone! on given name should we include our middle name (mom's maiden name)? in the application form is stated there Given Name (as shown on your passport or travel document)

passport details :

lastname/surname : dela cruz
first name: juan
middle name (moms maiden name) : aguirre

application for TRV which is right - dela cruz, juan aguirre or dela cruz juan ?


and on #9 Country where applying - im citizen here in the philippines and i live here. So ill put philippinesn(country), citizen (status). my question is the 9B - From To ..since i born here in the philippines
the from should be on the date i was born (?) and how about TO (the end date of your immigration status)? what will i put? i put date today but theres a note there - must be greater than todays date...PLS HELP

Here's what I did with my application form:

SURNAME: DELA CRUZ
GIVEN NAMES: JUAN AGUIRRE

In the visa anyway, they will not put your middle name on it.

For item 9B just put a date ahead of the current date, like if today is 2014-05-02, you can put 2014-05-03

I am a indian currently working in singapore. I have a workpermit to canada and i need to apply for a visa. I was told that processing in singapore takes 6 months and was advised to apply in india which has shorter processing times. Here is the question
My current country of residence is singapore so put in singapore and write the from and to dates.
next it asks whether country where applying is same as country of residence, I selected no and put india as country but it asks for to and from date. Given that I am a indian citizen, i put my date of birth as "from" date, what should i put "to" date??
