Several forms are available in the net for CAIPS note request as follows :

1) IMM5475 E issued December 2004
2) IMM5563 B issued January 2009
3) IMM5476 E issued July 2011
4) IMM 5563 E issued January 2012

So updated and correct form is IMM 5563 E which can be downloaded from the link :

While using any form please check at the bottom left corner the issue date is given (year - month) so you will know whether you are using an updated form old dated form.

Could you please advise how to use the latest version of this form IMM5563 E?
I need to order CAIPS notes and I don't know what option to tick in Section B?
