For an applicant under 18 am confused about a couple of the forms:
In the instructions it states that the Family Representative form needs to be filled out if you are 18 or older BUT in the document checklist it states that it needs to be filled out for a minor traveling alone as well. Does the family representative form need to be filled out for someone under 18?? If so does the parent then sign it?
For the Use a Representative form does the parent sign this? There are no instructions if you are representing someone under 18.
The other forms are clear as they state that the parent is to sign if under 18.
