If you're talking about a "poison pen letter", then what would happen is the person being sponsored will most likely be called for an interview, if the allegations are somewhat credible. From almost all the cases I've seen, the applicant almost always gets an interview after someone calls in to report marriage fraud.

Although it is rather annoying to have to go through the extra wait time due to an interview, at least it won't be a problem to ace the interview should the allegations be false

Agree. If someone sends CIC or the visa office a letter stating your marriage is fake, CIC will investigate. If the accusation is plausible, the visa officer processing the file will probably ask you for more proof the relationship is genuine, and the applicant will probably have to go to an interview. However, they get lots of letters like this, and they realize that the letter might be written because someone is angry with the couple, or just mistaken. The PR application will not be refused just because of this letter.
