Yes. They may send it back as incomplete, or they may send a new IMM5669E to you and ask you to complete it again.

You mean a piece of paper you made yourself, because your info didn't fit in the IMM5569E? Back when I got my work permit I've sent one without signing and there were no issues. I believe that it should be the same with the PR application, but you may want to confirm with the CIO.
