Yes you will need to add OINP as a recipient of your WES assessment.

Its pretty much the regular format but the important thing is that the duties listed on it should match your NOC code's duties and your designation.

I am not able to see my profile in https://www.one-key.gov.on.ca/iaalogin/portal/menu portal . it was there and last status was Assessment complete and I have received a mail from Gov as well about the last updated status. Now can not even see my profile .

Can someone let me know what might be the issue and to whom need to contact.
