How would I answer the residence questionnaire?

Hello there,
Can anybody please help me about the residence questionnaire? I just received a RQ last May and I want to know if you will answer in the questions since the day you became a permanent resident and just provide documents that cover the 4 year period as stated in the form?Thanks a lot.

Thanks a lot. My question is should i answer the questionnaire on the day i became a permanent resident which is nov. of 2001 or only within the last 4 years that i apply, period of 2008 to 2012? i find the question quite confusing such as no. 10-where would your family lived since my arrival date in canada? and they have from and to.....from is the date i became permanent resident?

Answer for the first question when did you become permanent Resident which is Nov 2001. You have to put the same date Nov 2001. Its the date of your PR and time when you came to live in Canada first time.
You only need to provide them all the residency documents,evidence for the period of 4 years which is 2008 to 2012 in your case.
