How to obtain Quebec Healthcare Claims Records

If you have been admitted into a hospital you can request a copy of the summary sheet in your file at the hospital, o the CLSC, or the clinic were you were seen by a doctor. You can also request from the pharmacist a record of all the Rx´s you got from him/her for every year.
This is true, but the CLSC, hospital and clinic visits should all be mentioned in the RAMQ profil medical. So it's not necessarily to double the trouble.FL1040 said:

I know this is an old post. But if anyone's in need of the health claim summary from RAMQ one day, here is the link to the form from RAMQ site. You can either fax it and send it to the address after filling out the form. (as of March 2019)

Thanks! Any idea on what required documents need to be submitted along with this form to RAMQ?
As far as I know you don't need to submit any document, unless you are a representative, in which case you must attach the documents attesting that you are acting on behalf of the person concerned as the form specifies in section 2.
