How to get upfront medicals for Super Visa?

Upon trying to book for an appointment for medicals, the approved physician near my location says that we need to get some form for medical test while I don't see any such form after completing application for my parents or don't see anything on IRCC website. Can someone please guide how to get upfront medicals done for a Super Visa?

Hi @Mirapakay , Can you please share if you needed any document when your parents went for the upfront medical? Did they ask for application number or any form? Or did they receive anything to submit with the application?
