Dear PMM
I have gone through the instructions but still confused regarding IMM5563B.
The first line or Requester's reference no. and AITP file no. (is for cic office use I believe).
In the second line Surname, First name and DOB. (Who's details are to filled here? Is it my details or the person in Canada who is requesting caips notes on my behalf?)
The third part there are two columns on reads Access to information act and the other is Privacy act. This is confusing as I do not which column and the subsequent subsections are applicable. I just want caips notes not the photocopy of my entire file. Please advise which one is applicable and what is to be left blank/ticked in the subsections. I have read in other forum that people ticked the wrong boxes and got the entire file photocopied and sent to them.
In the fourt part I tick CAIPS, Embassy name and visa file no. (that is ok)
In the fifth part I tick (as the person is in Canada)
Name of requester +Tele+Add+Sign (this I believe should be canadian person's details to be mentioned)
Please advise on the above. Your help is much appreciated.
