Don't the MR forms come with the UCI numbers? Since I'm the PA, only I have a UCI number and I expected my wife to receive a UCI number of her own when the MR forms came through.

How can we go to a DMP without the forms from the VO and ask them to use a blank form without the UCI number for my wife?

my medical status was pending as my wife did not take part in only the X-ray examination due to her pregnency (others medical examination has been completed).
Now Our baby is born and I have sent all the necessary docs to the VO.
The VO requested only my baby's medical and did not give any instruction about my wife X-ray examination.
What should I do in this situation?
Please help me in this regards.

U'll need 2 forms:
1. Form IMM 1017 (05-1998)E; and
2. Appendix C – Medical Instructions for FSW.

Here's the guideline/s:

A. When you and your family members go for your medical examination, make sure you have these forms completed for each person, along with the following items:

1. Passports for yourself and your family members, plus a photocopy of the bio-data page of each passport. If a passport is not available, provide an official identity document bearing the photo and date of birth of the family member;

2. Eye glasses or contact lenses, if you or your family members wear them;

3. Previous medical report(s), if any. You must tell the examining doctor about all past and present medical problems and conditions, and provide the doctor with any available documentation such as physician's reports, treatment or prescriptions; and

4. Five recent pictures [taken within six months preceding the date of the examination] for yourself and your family members (see section on photos in the instructions on how to complete the Application for permanent residence).

B. Make an appointment with one of the doctors identified on the enclosed list. A Medical Report, Section A (form IMM 1017) will be completed at the doctor's office for yourself and each of your family members undergoing the medical exam. A photo of the person to be examined by the doctor will be affixed on both copies of that person's form. This form is used to match your medical file to your immigrant file.

C. The doctor will sign the form and mail it to the Canadian Regional Medical Office (RMO) with the results of the examination.

I never had TB or been treated for TB. So i dont understand what is going on.
Do i need to get seperate check up and send the report.

Usually we need to do our meds thru the forms [IMM1017] supplied by our VO [the concerned CHC]. However, there could be reasons for doing the same WITHOUT them. Eg. the forms are lost, the CONsultant is withholding them, forms r somewhere & u r somewhere else etc. In such case, we 'can do the meds w/out the forms' too. Here's how:

1. Email CHC again to re-issue u the IMM1017 [med forms]... if time permits.
2. If not, Contact a DMP for an appointment.
3. Inform the DMP, in advance, that u want to do the meds "w/out" the forms.
4. They can get your meds done by 'creating' those forms* for u.

*Note:
- All DMPs have those 'blank'* forms.
- All they need is an 'authority' to do so. If u have the CHC's email - it would suffice.
- If u don't have an email from CHC, yet the DMP'll be happy to assist u [read they want the money].
- They'll need your File No. + 2xPhotos to prepare the forms [specific for u].
- U need to show the DMP your original Pps & pay the DMP's Fees [of course].

Important: If u do not get the CHC's nod to do the meds, remember the meds expire by 1yr & your visa/s will be valid till that period 'only' [except if anyone of your dependent's Pps expire before that, then it'd be earlier]. So, doing the meds early could be risky - as it might offer u lesser time to 'land'. If that's Ok with u, plz be free to get the meds done earlier.

I got MR on 15the Feb and received medical form online on 18th n get my medical done on 23rd Feb 2013.
I got married on 23rd Nov 2012 and sent complete file of my spouse( including his PCC,fee,educational documents) on 28th Jan.2013. Firstly I presumed that I got medical for both but waiting for 2-3 days we been to CHC delhi and found that they sent me MR on my single status only and have not updated my martial status as they had not opened my spouse's courier file and had not en cashed the fee yet.
My query is should my spouse too get his medical done now without form or we have to wait for his medical form which no idea when CHC gonna issue .?? Will VO take long to issue MR to my spouse as they will review his whole file yet...as my consultant written them that we are going submit rest documents such as my PCC, RPRF only when they will issue my spouse's MR..!!
I am in a opinion that we should sent CHC all docs including spouse's MR(My MR would have reached there) and our PP's so that when they will see all supporting documents then spouse's file can be processed early as I do not claim any point from my spouse..!!
Please suggest what shall we do ..??
 
Dear Qorax,

I had received email from CHC on 15 Febuary 2013 to submit the required documents of new baby after the delivery. My wife delivered baby girl on 04 March 2013 and I have sent all my documents to them on 23 March 2013. Our medicals expires on 23 July 2013, and CHC mentioned that will be the visa validity whenever they issue visa. Now I am waiting for medical request for the new born. Can I wait for CHC request or go ahead and approach DMP to do the medical by showing the deadline?

Please suggest me and share valuable knowledge.

Dear Quarax, my case is also some what similiar....

i was blessed with twin daughters in septmeber and i intimated CHC in Oct. regarding this, but got no reply. Meanwhile, In Nov. i got medical request without printed forms. We did ours as well as our daughter's medical with the DMP on Plain forms in December. In Dec. i send complete package with Landing Fees, my daugther's application fees, Daughter's Birth certificates, Medical Reports (including daughters) PP's of all of us ( including daughters) and a request letter to include them in my application. Meanwhile, my wife (Main Applicant) was subjected to re-medical by DMP as her XRay showed a scar. She had to undego sputum test (Report=Negative) as well as repeat Xray, this whole process consumed 2 & 1/2 months. now finally, according to our DMP he has send the medical reports to CHC-one month back.

this week, i have received an email & letter which goes like this:

DEAR _______: This refers to your application for admission to Canada. In order for us to continue the processing of your application, we require the following: Ø Proof of medical completion for dependent children(medical form and instructions attached) The above requested need to be submitted by 21/04/2013. Failure to submit them within that time period could result in the refusal of your application. Please quote our complete file number on your correspondence and address your correspondence to the High Commission at the address indicated below. PLEASE RETURN THIS FORM WITH THE REQUESTED DOCUMENTS. Sincerely, Immigration Section

I am confused as i have to take my kids again for re-medical to DMP or they just want photostat copy of Medical Report...

My passports are already with them since june 2013.

What do u feel post strike will i get my visa before nov or i will i have to go for remedicals...

totally messed situation right now...

I got medical forms from visa office on 10 sep 2013 and I have to submit it with in 60 days, Is there any request form or i should email them to delay my medical??
The reason for delaying the medical is that my PP will expire on 14/07/2014 and i only have 9 months remaining on my PP its mean that i have to travel on that time frame, Also my current company doesn't allow my to leave my job before 15 months.

1. They gave you 60 days, if you don't have the medical, you will get a letter saying "that it appears that you are no longer interested in Emigrating to Canada, if we don't receive your medical results in X days, we will close your file" Your choice, you either want to emigrate or you don't .

Good news is that you don't have to delay your medicals for this. Please proceed for medicals with current passport and then apply for new passports as soon as you are done.

The passports you submit for visa, don't have to be the same as the ones used for medicals.

It is very common and lots of ppl have done this.

PMM! they don't close the file of their own, rather involve you by saying that " if you are no longer interested in pursuing this application, please forward a signed request for withdrawal to this office and we will withdraw your application."
