How to correct citizenship certificate

My First name was wrong, it comes with my middle name.

First Name : Dianne
Middle name : Velez
Surname : Diaz

on the cert it is mentioned

Surname : Diaz
Name : Dianne Velez

When I applied for the passport, the staff told me to correct the name it is supposed to be Dianne only. She mentioned that I have 3 months to correct it.

can anybody guide meÉ

Hi Dianne,
Mostly CIC copy the information from a previous document i.e. use the same information found on the landing document to complete the Citizenship Certificate. Then;

1) If CIC made an error it is here:

Can I replace my citizenship certificate if there is a spelling or other mistake on it?

Yes. We will replace your citizenship certificate if:
we made a mistake in processing it, and
it has been 90 days or less since you received your citizenship certificate.

How to request a replacement if there is a mistake on your certificate
If it has been 90 days or less since you received your citizenship certificate, send us:

the citizenship certificate with the mistake,
a note explaining what needs to be fixed,
and
if your request is urgent:
write “Urgent” in large, dark letters on the envelope,
include an explanation of why your request is urgent, and
include any documents that support your explanation.

We will replace your certificate free of charge.
Where to mail your request

If you live in Canada, mail your request to:

that is why i am wondering also. but the passporting said it is wrong that my name should be only name and it doest not appear the middle name. I dont know why she said it is wrong and i need to correct it. She said that the surname should be DIAZ and Name should be DIANNE only!

Maybe the reason why is

My single name is

Name : Dianne
Surname : Velez
Middle name : Char


when i got married my name changes to

Name : Dianne
Surname : Diaz (my husband surname)
Middle name : Velez

is Velez supposed to be in the surname?

it is supposed to be

Name : Dianne

im confused!

I think this depends on how you use your name and how it looks in your passport from your original country. You want all your documents to say the same. Did you also change your name in your country of origin? Did you change your name after applying for citizenship?

As an aside note, if you live in Quebec, things are very complicated with middle names. If you applied for your Canadian passport in a Quebec office, I would take what they say with a grain of salt.

well my passport in my country is like this

Surname Diaz
Given Name Dianne
Middle Name Velez

Coz in my country when you get married you will carry the name of your husband.

I change my surname since 2008 so all my legal paper PR LANDING FORM DL is Dianne Diaz aside ofcourse from Quebec Health card i used Dianne Velez

I don't know what the passport canada lady is talking about. The name format of the citizenship certificate is:
Family name:

Given name(s):

Note the (s), which means there could be one or more given names here.

Surname : Diaz
Name : Dianne Velez

To me, I understand this as first name = Dianne, middle name = Velez.

Why would the Canadian gov't issue your certificate with just two names if indeed you are using 3 names? I suggest calling CIC call centre, ask them about it and perhaps get them so send you actual information about this. Or call Passport Canada and hopefully, you get to speak to a different agent.

well me to i got confused when she said that it is wrong and Velez shouldn't be in the given name. bu actually she didint say anything about my husband certificate, he also have the same format of given name : name + middle name

People can be mistaken sometimes but for someone working in a powerful agency as this, such mistakes can be costly for applicants. Is she trying to say there are no Canadians with 3 names (first, middle, last)on their passport??? May be ask her to show you an actual Passport Canada policy document where it says such.

Well, now I doubt may be this has something to do with the way you filled in passport application form. As there are three boxes (PPTC 153) for surnames in, either one of the box is filled in wrong or other side/pp officer is getting confused.

I think from the strictly legal point of view you're OK (but of course, I'm just an ordinary person, not a legal expert). Do you have any idea if the Passport Canada employee spoke Spanish? I can see why some people from certain Spanish speaking countries would say that Velez shouldn't be a middle name.

Of course, the right course of action here regardless of any of my speculations is to follow Senorita Bella's advice and call CIC/Passport Canada just to make sure.

There this problem in Quebec is that they don't understand middle names in the same way as the rest of the country. If you declare a name as a middle name in Quebec, it won't show up in any document other than a birth certificate. If you want it to show up, you must declare it as part of your first name, and then it will show up absolutely everywhere. There is no middle ground.
