I'm not sure what you mean by apply for medical reasons. If you are asking about permanent residency - there is no medical program. If you are asking about a visitor visa so that you can come to Canada for treatment, you will want to obtain a letter from the hospital / doctor who is going to treat you in Canada and include information about what it will cost for this treatment. You will then need to show that you have a sufficient bank balance to pay for all of your treatment in Canada - as well as your flight and accommodations here. Keep in mind that as a visitor to Canada, you will not be covered by the health care system and will have to pay for everything yourself.

AS is a degenerative autoimmune disease. You can’t move to Canada for life to receive treatment. The cost of medication is also very high and you may already be disabled so passing the medical inadmissibility criteria may be difficult if you qualify for PR. If you qualify based on work experience, age, education/skills, money and language scores you should apply but be prepared to argue that your treatment will only cost under 20k per year..
