I (sponsor) asked for notes and i got an email from IRCC asking my spouse to fill up a document to give consent to realease his informations to me.
He filled up the document and we want to email the document to IRCC.
My question is, can my husband just scan the signed document and email it to me so i can email it to IRCC? Or how can he convert the document in pdf file?

This is part of the email I got.

There is section a-d
Section a
His informations. And he signed and put the date

B-if he wants to get access to my informations (I didn't fill it up because I don't think we need it since I am the sponsor. We just need the notes for his application to PR.)

C- my informations because he is giving me consent.

D- under 18 (dont need this)

http://www.cic.gc.ca/english/pdf/kits/forms/IMM5744E.pdf

Section A: You must clearly write your name and date of birth (requester)
Section B: if you are also seeking the information of your spouse or common-law partner in your ATIP request, you must indicate their name and signature (your spouse goes here)
Section C: You must write your designated individual's contact information (this is the representative it appears)
Section D: if the applicant is under 18 years of age, the parent or guardian must complete this section (N/A)

From other correspondence I have had with them, they really only need sections A & B. If you are using an immigration lawyer or consultant, they fill out Section C.
